CADET & SENIOR APPLICATIONS
Cadet & senior applications will be accepted if postmarked from June 11, 2008 through July 6, 2008. Applications after this date or at sign in will NOT be accepted.

CADET APPLICATION PACKET
Must be postmarked no later than July 6, 2008.
The following 9 items (8 documents and 1 check) are required for all cadets applying to Encampment. DO NOT SEND TO NATIONAL HQ
- CAP Form 31 - Application for CAP Encampment or Special Activity. Two copies, signed by the unit commander; page 2 not required; photo highly encouraged.
- CAP Form 60 - Emergency Notification Data (In order to reasonably accommodate special dietary needs, please note clearly any dietary restrictions for health, cultural, religious or other reasons on this form. Failure to do so will negate ANY accommodation).
- Child’s Release and Hold Harmless Agreement (MUST be signed by parent and witnessed by a second adult)
- Medical Emergency Parental Consent (MUST be signed by parent and witnessed by a second adult).
- DOD Flight Form (Air Transport Agreement) (MUST be signed by parent and witnessed by a second adult).
- CAP Flight Release Form (Air Transport Agreement) (MUST be signed by parent and witnessed by a second adult).
- Pennsylvania Wing Cadet Code of Conduct (MUST be signed by cadet, parent and squadron commander)
- A 3X5 index card with the Cadet’s Name, CAP ID, Parental/Guardian Name, Parental/Guardian Email Address, Parental/Guardian cell and home phone numbers. Alternate Adult contact cell and home phone numbers.
- A check must made payable to “PAWG CAP” for the registration fee:
- PA Wing Basic Cadets (not on staff): $175
- Out of PA Wing Basic Cadets (not on staff) : $225
- If a PAWG cadet, you can instead pay by credit card by going to the CAP Region/Wing link ON-LINE.
All the above documents and check(s) (if not paying directly by credit card) are to be mailed to:

To be completed prior and brought by the cadet for sign-in at the Encampment:
- Two updated copies of the CAPF31, only if any information has changed since registration (Squadron Commander signature not required.)
- Participation letter (from the Squadron Commander)
- Medications (if any, see Medical)
- Encampment Required Equipment Packing List - This list must be completely filled out for presentation when signing in. Failure to have form completed and have all necessary items will prevent you being accepted at Encampment.

An additional item and additional check is required for all cadet staff requiring shirts and/or covers, whether free (if entitled) or to purchase. If purchasing, an additional check for total amount of purchases must be included. Please see “T”-Shirts and Covers as these items must be ordered in advance. They will NOT be available for purchase at ENC.
The Shirt and Cover Order Form and a second separate check made payable to “Lt Col Willa Hayes” for cost of the “T”-Shirts and Covers is to be mailed to:
